To make an appointment, call us at (330) 682-6070. We offer timely scheduling, most times within one to two days. We offer on-site x-ray facilities and orthotics lab. All of our orthotics and braces are custom-made here in our office. We have an orthotics technician and pedorthist on site.

For your convenience, you may print and complete the following forms and bring them with you to your appointment. Also, please bring your insurance card (s) and photo identification. We ask that all patients under the age of 18 be accompanied by a parent or guardian.
